Reporting and investigations play a pivotal role in disease prevention and control. The Missouri Department of Health and Senior Services (DHSS) has compiled a list of
reportable diseases and conditions that are, by law, required to be reported to the local health department by health care providers.
Reporting these diseases and conditions is done using a
CD-1 form. After reporting, disease investigation is initiated to identify possible sources and ensure that all necessary prevention and control measures are undertaken in a timely manner.
